road previous to that junction is a dual carriageway apparently, so brain could be in dual carriageway mode. and we don't know what other traffic was around on that approach at the time...
I had a look at the Google streetview. There's two signs, one two lanes into one, and the other 2-way traffic. You then go along the road for a bit and it then splits into two. I still think it's driver error, but not what she's got done for. Driving without due care I think matches better than what she got (which is a lot worse). She missed the signs, and when faced with an oncoming lorry dove into oncoming traffic rather the ditch (not judging on that, making the right split-second choice is 50/50).
All that said, I would let her off. Life passing before your eyes and almost dying should be more than enough to remind one to pay close attention to any and all signs.
