Nuts and bolts rattling in a can sound

One year ago, it was an intermittent and occasional problem. Now, it's become much more frequent. What happens is that occasionally when I drive off, after a few minutes I hear this horrible high frequency rattling sound that again sounds like a can of nuts and bolts being shaking very quickly. It only happens while accelerating at about 2500 rpm and then goes away at about 3500 rpm if I keep accelerating. It seems to occur more often within 3-minutes after a cold start but it can also occur two hours into a drive. Had it into a mechanic and the computer said that the throttle body was malfunctioning. It also said that the ambient temperature was 140C. and that the coolant temperature was 30 C. Yeah, right. We agreed not to spend $700 to replace a throttle body that was not clearly indicated by a reasonable fault code. So, all that having been said, do any ideas as to what's going on?
 
rattling makes me think it would be a catalyst or muffler problem, but the mechanic should have been able to figure it out if that were the case... hmmm
 
Several possibilities:

VANOS
DISA valve
Pre ignition

Search on each will provide the info you need.
