Probably off a trailer , it happened to me once , mate driving , I'm in the passenger seat and something caught my eye . We stopped and sure enough one of the wheels had come off and ended up in the field beside the motorway !
Worst thing was , driver never felt it even though we had a stock car on the trailer ! It happened to a few other people I know back in the day on the way back from races , one guy lost the whole trailer and car combo :driving:
 
Not the first time I've seen that video but don't think he did survive. Pretty sure he died instantly speed that wheel was coming at is a spine breaker
