Gap Insurance: Which (UK) Provider?

A few topics discuss whether or not to buy GAP Insurance, but there's little on which provider. Although the overall cover is broadly the same for most of them, they differ in important details. For example, currently top of my list is car2cover.co.uk because theirs is the only policy (of the ones I've checked) that meets all of:
- is fully UK FSA covered
- will still pay out if I decline my insurers offer of a replacement car and accept a cash settlement instead
- is transferable to a subsequent car (either following a total loss claim or if I just change car)

My question is: what differences in policy cover swayed your choice of GAP Insurance provider, ignoring differences in premium? I'm wondering what other bits of small print I should take into account.

The second point above is particularly important I think: most GAP policies explicitly refuse to pay out anything if you decline your insurers offer of a replacement car. It's almost certain I wouldn't want exactly the car my insurer offers, I'd want to spec it differently, or buy new again (rather than "of equivalent age/mileage" etc) - either of which would render the GAP insurance invalid. Has anyone been in this position and what was the outcome (ie could you persuade the GAP provider to pay-up)?

Also, why is GAP Insurance only (as far as I can see) offered by niche insurance providers rather than by the mainstream car insurance companies?
