When I fitted my new fog lights, I was only aware of one "breath" hole and that is situated on the top of the light under a small black cap.
To prevent condensation build-up, I (rightly/wrongly) siliconed these holes. So far no condensation
If you already have condensation, many people drill very small holes on the underside of the lens.

I drilled 3/4 holes on the underside of the plastic 'glass', I've had fog-free fog lights since.

It's all down to air circulation inside the lamp probably best to drill small holes I top and bottom diagonal corners so air circulates, maybe stick some open gauze on the top one so air can get in but nothing else. Seems to remember doing this on a Range Rover to cure fogging on that.
