Faulty Handbrake & Brake Discs - Need Replacing?

I can pull the handbrake up OK and it feels normal, however sometimes it holds and sometimes it doesn't. These are my brake discs

I'm thinking the discs will need replacing but could it just be a case of adjusting things? If I do have to replace the discs do I need to do front & rear at the same time?
 
The handbrake shoes and adjustment etc are inside the rear brake rotors, the hub part. Can't be seen from the outside.The state of the rotors that you can see don't affect the handbrake really.

It is more likely the handbrake mechanism near the driver is worn.

Yeah, had the car coming up 5 years, at the stage where I need to start investing in it or move it on for someone else to do likewise
