Do you think this adapter would work with the OEM iPod adapter?

Also would this effect me using the factory fitted Bluetooth hands free?

I want to say yes, but as far as I know, most phones only connect to one device.

However, the audio would route through the device and then the microphone is used on the device.

What was said above - most/all phones will only pair with one device at a time.

I've got one of these - http://www.amazon.co.uk/Belkin-Car-Audio-Connect-AUX-Bluetooth/dp/B004FNYQZ8/ref=sr_1_1?s=electronics&ie=UTF8&qid=1379258137&sr=1-1&keywords=aircast - which does calls and audio, it's wired in via the aux.

Call quality is on a par with the built in, possibly a little better I think as mic is in a better place (hard to tell - it's the people the other end who get to hear what I sound like!). To be honest, I rarely use it for calls as almost always got the roof down!

That's simply not true...

Firstly, the pairing is the act of setting up connection / authentication information. I don't think there is a limit to the number of pairings.
Secondly, you can have multiple concurrent active Bluetooth connections with most modern smart phones.

For example, I use Bluetooth to establish 3 active connections on my in car setup.

EDIT - just re-read the OP question, and the answers are sort of correct (sorry). You will struggle to connect to two devices of the same type at the same time. Although if your device is smart enough to differentiate between A2DP and handsfree profiles, it could be do-able. Not that familiar with IOS I am afriad.
 
I've already got the OEM iPod kit fitted and the factory blue tooth, so would be pointless fitting something else as both work very well. I just like the idea of wireless sounds.

I've found it for £15 on eBay, might have a punt when I get back off holiday.

Well if you already have it working in that respect, ditch the ipod cradle and use an airport express.

stream music over wifi (ariplay), and bluetooth for handsfree calling.

Even better quality with airplay than bluetooth. It's a win win.

Have you hardwired it in or plug in the cig lighter each time?
