Do you have a pic? I think mine looked ok when I check, its the thing that has a nipple coming out of one side right that should be all the way in so it looks like there's a hole one side?

Ah, I've had that a couple of times. There's a spring and tab under the lever which you are supposed to engage to take tension off the cable when you're adjusting it. On both occasions for me that tab was engaging every now and then and of course making the cable go slack. I assume something gets weak in there?
If it happens again, don't move anything, whip up the gaiter and see if the tab has engaged with the slotted disc at the end of the spring. If so, ping it back with a screwdriver.

When I adjusted mine a couple of weeks ago I just did it though the hole on the hub and did it until it wouldn’t move, wheels on let it down tried to pull away with handbrake on it stalled, so I guess I did it ok.

how high up was your hand brake going up pre tightening.

It was quite high and the teeth were worn and only the first two worked, so just adjusted up for those and I got it good enough to pass the MOT two weeks ago.
