I have a friend (yes I know) who works for one of the big insurance companies (BGL). Her job is to input claims data into a computer system. That is all she does, along with tens of other people. The massive computer then deciphers the information into risk criteria and hence premiums.
The humans just feed the machine, the machine makes the decisions. Classic case of "computer says no". No wonder it's all a bit random.
All good things come to those who wait. I'm really impatient which explains a lot.

The program the computer is using to decipher the info was still programmed by a human though
